• No Providers who worked for Envision within past 12 months and that live within 75 miles of the site 
• No Providers who worked for this site or an affiliated site in the past 24 months  
• No Providers that worked at this site or any Envision site that is within 75 miles of this site  
• New Candidates that have never worked for Envision must live outside the 75 mile radius of this site. 
• No Providers will be accepted who currently reside or have a primary residence in the counties of Broward, Palm Beach, or Dade County. 

***Please note that a call interview/Zoom Meeting with the Site Medical Director (SMD)/Clinical Leadership at the site will be required. Contact phone number and best time to contact over the next two weeks must be included with the presentation. Not including this information will delay the presentation from being presented.

Temp/Expedited Privileges: Yes, 90 days (CLEAN FILES ONLY)

Types of Cases: General, OB/GYN, Outpatient, Neuro, Orthopedics, Interventional Radiology, GI, Robotics, Pedi, Urology, Vascular and Thoracic Surgery, and Cardiac.

Majority Case Type: General, Neuro, GI

Average Monthly Case: 2500

Required Procedures: Spinals, Epidurals

Life Support Requirements: ACLS, PALS

Shift Days: Monday – Friday, 8 hour shifts (7a-3:30p)

APPs will be required to work 2 weekend days per month. Does not have be consecutive. Weekend shifts will be 12 hours (7a-7p).
